This weekend the Street Medicine Phoenix (SMP) team, packed more than 140 Care Kits for Phoenix’s Homeless population. In addition, to providing a Care Kit packet to each homeless individual, a health needs assessment will be conducted, to learn more about the specific services (i.e., health, transportation, housing, etc.) that each homeless individual needs, in order to overcome personal obstacles and societal stigmas in their pursuit of happiness, well-being, and a home.
SMP’s mission is to provide access to quality, individualized health care and meet the unmet needs of Phoenix’s homeless population living and sleeping on the streets.
